Definition
Case management at SCS is a supportive, person-centered approach designed to support individual goals and navigate the steps needed to achieve them. At the outset, a screening process is completed to determine the most appropriate level of support for each individual. Based on this, participants may receive case management or brief case management. Case management focuses on supporting individuals across multiple domains of need through collaboration, planning, advocacy, and connection to community resources to address goals.
In contrast, brief case management provides short-term, targeted support, focusing on fewer or more immediate areas of need. If case management is not required at this time, resource management support will be offered, providing information, referrals, and guidance to help individuals access available and relevant community supports. Together, these approaches ensure individuals receive the right level of service to promote independence, well-being, and meaningful outcomes within the Ottawa community.
A Case Manager can:
- Assist in developing and implementing a service plan with SMART goals that promote development, safety, independence, and the ability for the child or youth to remain in the family home.
- Facilitate and take part in person-centred planning meetings to support coordinated service delivery and life transitions.
- Encourage and support person-centred planning with meaningful involvement of the child or youth whenever possible.
- Help families build and strengthen both informal and formal support networks.
- Enhance parent and caregiver understanding of available services and support system navigation.
- Provide information, coordination, and service navigation, including assistance with referrals and applications when appropriate.
A Case Manager will not:
- Provide crisis intervention or after-hours support; case managers are not designated crisis responders.
- Attend meetings in place of parents or caregivers.
- Communicate or collaborate with partners or professionals without the consent of the parent or caregiver.
- Support or take part in services or processes beyond the scope of the agreed service plan, especially those involving legal or custodial matters.
- Guarantee access to funding, whether short-term or long-term.
- Arrange or provide out-of-home placements or group home care.
- Recommend, deliver, or oversee clinical or therapeutic services.
- Confirm or secure eligibility for DSOER or adult services.
Your relationship with your case manager is voluntary, based on your active participation in your plan. The worker facilitates goal oriented planning with time defined supports. The person/family is always an active participant in all decisions.

Eligibility:
- Under the age of 18 years
- Live in Ottawa
- Have a confirmed and documented diagnosis of a developmental disability and/or autism spectrum disorder.

To register for case management services, a parent/guardian must complete the following form, and include the required documentation.
The registration form will be reviewed by Service Coordination Support (SCS) to confirm eligibility and outline next steps.
