The roadmap provides an overview of our services by age groups to help individuals and families understand which service they may require. Information includes a definition of each service with their eligibility and a link to learn more.

Main Services Provided at SCS by Age Groups

Image description: See Map Legend below for acronym definitions. Our main services for young children are Children’s Case Management (CCM) and respiteservices.com in Ottawa (RS). As the child gets older, around age 14, it is time to start thinking about the transition to adulthood. Developmental Services Ontario (DSO), along with CCM and RS, play an important role in this transition. When the person turns 18 or older, DSO continues to play an important role in their transition to adult services, which may include Adult Case Management (ACM) and RS.

CHILDREN’S CASE MANAGEMENT

Available in Ottawa

Case management at SCS is a collaborative, inclusive and individualized process. We work towards goal setting, planning, advocating and exploring available community resources to support the achievement of personal outcomes.

ELIGIBILITY:

  • Up to the age of 18
  • Resident of the City of Ottawa
  • Diagnosis of a developmental disability and/or autism, supported by psychological assessment

DEVELOPMENTAL SERVICES ONTARIO EASTERN REGION (DSOER)

As a service provided by SCS, DSOER is the access point to adult ministry-funded support and services available in your community.

ELIGIBILITY:

  • 16 years of age or over, and reside in Ontario
  • Diagnosis of a developmental disability, supported by psychological assessment
  • Want to access services funded by the Ministry of Children, Community and Social Services at 18 years of age or over

ADULT CASE MANAGEMENT

respiteservices.com in Ottawa is a secure website with tools that match you with Direct Support Providers who can provide you with respite.

ELIGIBILITY:

  • Complete registration and receive approval
  • Find and hire a Direct Support Provider
  • This service is available to children and adults with developmental disabilities and/or autism in Ottawa.
