Your Roadmap with SCS

Main Services Provided at SCS by Age Groups
Image description: See Map Legend below for acronym definitions. Our main service for young children is Children’s Case Management (CCM). As the child gets older, around age 14, it is time to start thinking about the transition to adulthood. Developmental Services Ontario Eastern Region (DSOER), along with CCM, play an important role in this transition. When the person turns 18 or older, DSOER continues to play an important role in their transition to adult services, which may include Adult Case Management (ACM).
